I, the legal parent/guardian grant permission for my child to participate in any and all activities at the Open Door Church 2025 Sports Camp. I recognize and acknowledge the inherent risks that these sports activities may present for my child. (Please select Yes or No)

In consideration of my child's participation at the Open Door Church 2025 Sports Camp, I hereby grant permission to Open Door Church the use of photos and recordings of my child taken during the sports camp for the purpose of promotion and church publications, exhibition and distribution of Open Door Church material. I understand that no royalty, fee or any other compensation of any kind shall become payable to me by reason of such release and use of any photograph.
